Thank you for choosing to have your surgery with us at Salt Lake Surgical Center. It is very important that we contact you prior to your procedure.  A business office patient representative will contact you to verify the information we have received from your physician’s scheduling staff. We will confirm any insurance coverage (if applicable) and any fees that you may be responsible for before your date of service.  You will be asked questions regarding any prior surgeries and your past medical history. Your physician’s office staff will give you an admission information packet. Please complete the questionnaire, current medications/allergy and bring it with you on the date of service if you have not already mailed it to us.

The federal government requires that we inform you of three important issues:

1. Your rights as a patient at a healthcare facility. Enclosed is a description of the Patients Rights and Responsibilities.
   
2. The Salt Lake Surgical Center policy on Advanced Directives. As your procedure here is elective, the Surgery Center does not honor advanced directives.  In the event of an emergency that necessitates your transfer to a hospital we would either forward the copy you bring to the center or provide the information you provided to us to your next care giver.
   
3. The Salt Lake Surgical Center is a Surgical Care Affiliates surgery center with 14 limited physician partners.

You must be pre-registered prior to the day of surgery, federal regulations require that we speak with each patient or guardian. For your convenience, we have provided a Pre-Registration form. We ask that you please fill out the form and e-mail it to  amanda.eaton@scasurgery.com
